Services that extend primary care into the community and address an individual's ability to achieve personal health goals and positive health outcomes, by addressing whole-person health and connecting healthcare resources.

Acute transitional care

Services that provide a safety net after illness, injury or crisis to ensure not only recovery, but reintegration into the healthcare community and daily life, preventing further acute events.

Behavioral Health Support

Community Outreach and Prevention:
