When finished, read the "Third Rider Clinical and Observational Ride-Along Release, Guidelines and Waiver" carefully, fill it out, sign it and submit it with the form. If you're under 18 years old, you'll need a parent or guardian's signature, too. 

Third Rider Clinical and Observational Ride-Along Release, Waiver, and Guidelines 

When we receive your paperwork, we'll reach out to schedule the ridealong. We have five stations in Eagle County--Vail, Avon, Edwards, Eagle and Gypsum--you'll report to one of these stations at 7 a.m., 9 a.m. or noon, depending on the day and the shift. Ride alongs last for 10-12 hours. If you have a preference for station, please let us know and we'll try to accomodate that request.
